The Summit focuses on the importance of collaboration across the care continuum, challenges and opportunities across the policy landscape, best practices and innovative partnerships, and opportunities to strengthen coordination between primary care and oncology care providers. Interactive sessions will explore the roles of primary care and oncology providers across the care continuum from screening and risk reduction through survivorship; the impacts of primary care disruptions during cancer care; information sharing across care teams; and the impact of payment and reimbursement models on collaboration.
